For many older adults, having healthcare coverage does not always mean having access to care. Finding the right provider, understanding a referral, scheduling preventive services, accessing transportation, navigating Medicare or Medi-Cal resources, and connecting with community support can become increasingly difficult as healthcare needs grow more complex.
The Healthy Aging Access & Navigation Initiative is being developed to help older adults overcome these barriers through personalized health navigation, resource coordination, warm handoffs, and follow-up. Rather than simply giving someone a phone number or referral, our goal is to help individuals successfully make the connection.
The possibility: a community where getting older does not mean having to navigate healthcare alone.
Through this initiative, RCBC Health is working to strengthen the bridge between older adults, healthcare providers, health plans, preventive services, and community-based resources—helping more seniors remain healthy, informed, connected, and independent.
Understand each individual's health needs, coverage, and barriers to care.

Check in to make sure the connection was successfully made.
Track outcomes to strengthen and improve the navigation process.

Our Civic Engagement Initiative creates pathways for residents to better understand local government, develop leadership skills, participate in community decision-making, and work together on issues affecting their neighborhoods.
